FICTITIOUS NAME SUBMISSION INFORMATION

Fill out the form of choice and email it back to This email address is being protected from spambots. You need JavaScript enabled to view it..  The cost is $30.00 and once we have ran the ad and it has been paid for we will email you the affidavit.

Below are the steps.

1. Fill out form.  - Print name as you sign at the bottom where it says signature (we don’t need the signature just your name printed as you sign).  Also if there is more than one owner we will need their name(s) then the percent of ownership for all parties.  If it is a business owning a business then the business name and the managing members name.   The address needs to be the business address that you provided Sunbiz.

2.  We will send you a preliminary copy to review. - Review it and if there is any corrections let us know.  If there isn’t any corrections then you don’t need to do anything, we will automatically run it as is.

3. We will email you the invoice for the ad. - The invoice will come from This email address is being protected from spambots. You need JavaScript enabled to view it. and you will be able to open the invoice and hit the pay now button to make payment with any major creditcard.

4. We will email you the affidavit. - Once the ad runs and the invoice is paid we will email you the publication proof affidavit for your records.
